Step 3 Outreach Services

We provide Outreach Services in Bristol, Bath, South Gloucestershire and North Somerset. Each support package is tailored to meet the individual’s needs and can vary from a few hours a week to a complete 24/7 service. All service users can access a 24 hour emergency on call service.
Support can be provided for all aspects of independent living including:

  • Daily living skills
  • Emotional & behavioural management
  • Medication and monitoring of mental health
  • Socialisation and much more.

These skills enable the client to clearly identify their progress towards independent living and to improve their quality of their life within the community. We endeavour to support individuals to think creatively and to find new solutions to past and present problems.
To compliment this service we also provide an accommodation finding service, which supports our clients in finding suitable and good quality accommodation. This involves undertaking a range of tenancy related tasks, which aim to build good working relationships with local landlords.

The outreach services are managed by staff from the supported living services (step 2) in that area. This creates a “core and cluster model”, ensuring clients are supported by staff from their local community and provides a social meeting point.

Supported Independence also offers a outreach service funded by Bristol Supporting People for adults with learning disabilities in Bristol and South Gloucestershire. This service offers between 2 and 15 hours per week of tenancy related support to clients in their own homes.
All service users can benefit from a wide range of social and educational activities including:

  • Music technology project
  • Allotment group
  • Art group
  • Holidays
  • Access to work
  • Creative writing courses

Kingsholm

Kingsholm is a block of 4 one bed flats, in a newly converted house near Southmead Hospital, Bristol. Support is provided to clients that need intensive support, but find it difficult to live in shared accommodation.
